Download the Pilot Library Evaluation Form and guidance for Gadgeteers, Summer Reading Challenge 2022.

All library authorities running the pilot model of the Summer Reading Challenge are required to complete this evaluation form.

Please Note: This is the pilot library evaluation form. Welsh library authorities and those taking part in the regular Summer Reading Challenge model will receive different forms.

Please submit your evaluation form by email to [email protected] by Friday 14 October 2022.

Send your evaluation form as an Excel spreadsheet and only submit one form per authority. We are unable to accept forms submitted as pdfs or Google sheets.

Please note: this is a locked resource for library authorities participating in the Summer Reading Challenge 2022. Libraries should contact their local Summer Reading Challenge coordinator(s) for access.
